Prepare for Your Visit
On your first visit to our office, our receptionist will ask you to
complete a patient information form. The questions we ask will provide
the doctors with essential data on your health and medical history.
You will also be asked questions concerning your insurance and billing
information. There will be times when we will have to contact you, so
please provide as complete information as possible. Periodically, you
will be asked to update your patient file in order to receive the best
care and service.
Insurance and
Billing
We participate in most insurance and HMO plans available in this area.
Should you have any questions about whether we participate in your plan,
a member of our office staff will be happy to help you. We file all
insurance claims as a service to our patients.
Health
insurance is designed to help you meet the cost of medical services.
In most cases, it is not designed to pay total fees. Each policy and
provider offers its own terms. Please be familiar with your coverage
when discussing any claim with us. If you are unsure of your policy
coverage, we recommend that you call the 1-800 number listed on the
back of your insurance card.
We
cannot hold another party responsible for payment of services that you
receive. This applies to insurance companies, as well as to other parties
who you may believe are responsible for payment of your bills. We will
ask you to be responsible for payment if you are not covered by insurance
plans for which we are providers.
Appointments
Office visits are by scheduled appointments. At times, unfortunately,
emergencies and delays are possible. Should we ask you to reschedule
your appointment, your patience is greatly appreciated. We make a sincere
attempt to adhere to the office schedule as much as possible.
If
you cannot keep your appointment, please notify us as soon as possible.
Financial Policy
We make every effort to provide the highest quality medical care at
a reasonable cost and will openly discuss fees and billing policies
with you. We urge you to discuss any financial problems with us so payment
arrangements, acceptable to both parties, may be made.
Referral Numbers
If your insurance company requires a referral for you to see a specialist,
please work with your primary care doctor to secure the referral number
before you arrive at our office. Following this procedure will help
you avoid lengthy delays waiting for the number once you arrive at our
office.
Prescriptions
Prescriptions and refills of prescriptions will be issued during normal
business hours.
Emergencies
If you are in an emergency situation and feel you need to be seen immediately,
please call 911. Our physicians are available and on call 24 hours a
day for emergencies.
